This is a list, so get ready.
1. Ads breaking the player. This happens to me with all ads, randomly. The ad will finish playing and the player black screens. After this the only fix (after I click the report ad for breaking the player) is to refresh the stream and watch the pre-roll ads again and hope it doesn't happen again. Maybe make the report function skip past it after the ad duration is supposed to be over, that way it's not exploited by people trying to skip ads, and also gives some counterplay from having to rewatch pre-rolls 27 times a day.
2. Ads disable all buttons on the player including the feedback buttons. Once again seems to happen randomly, occasionally in combination with the previous bug. Oftentimes occurs when my player is set at the stream volume and the ad is set at "Michael Bay volume". Solution is to wait for the ad to end at deafening volume (can't report using the player and it's ambiguous to screenshot this one) and if it black screens, refresh and hope it doesn't happen again. This is site breaking and needs to be fixed now, get testing on your end, because my setup has very few addons so this probably isn't a single person issue. If this isn't fixed expect an increase in ad block users as even I'm considering it at this point.
3. Stream to ad volume disparity. Typically I'll have the stream volume set so that I can clearly hear the streamer, who insists that his volume is as loud as he can set, but then the ads come through at a much louder than that volume. Perhaps you can add a function for streamers to adjust their volume site side so that they can manually adjust it in order to avoid this. This would give the streamers a more consistent volume which would make twitch much more friendly to those not using ad block. It would also put a damper on the typical, "oh ads, mute instantly", behavior that I find myself doing a lot when the volume jumps suddenly.
*Note that when I say black screen, I am referring to the location where the video plays, the player still has pause, play, and mute buttons visible, it's just that no video is playing.
