Finally, yesterday I opened a call using Microsoft's XBox Twitter support: https://twitter.com/XboxSupport
The technician that Tweeted back said that I should follow the instructions in the first link. However, the one important item to change is this: On the Xbox, in Settings, Privacy & online safety, ensure that Adult defaults is selected (not Custom which was what mine was set to). That solved it for me.
