Title and game won't change (ps4)

Whenever I stream from my PS4, the title is always Test and it always says I'm playing The Playroom. No matter what I've tried, nothing works. Is this an issue with twitch, PS4, my account, or something completely different? Is there any way to fix this? If you look at my past broadcasts, they're all the same title and game, even though I changed the title and played different games.
