Having issues with "Failed to load" while trying to stream

While trying to stream I'll get about half an hour of good coverage then get a blank grey screen with "failed to load" everyone trying to watch gets it. I can fix the problem by just restarting the stream but it happens about half an hour again anyway and really messes up the flow. This problem is appearing while streaming from my PC with OBS and from the Livestream capabilities of the PS4, also when the "failed to load" comes up on the screen the streaming tools still say I'm streaming quite fine and there is no problem. any assistance on the matter would be greatly appreciated.
